Collection of Personal Information:

During our work together, private information such as clients' names, addresses, contact information, and session notes will be maintained. Information will be securely documented and not available to others. Notes will never be shared but, on request and consent, a summary can be shared with the client and/or collaborative team members.

Method of Collection:

Client details can be sent by email or printed and mailed to the provided address.

Use of Personal Information:

The purpose of collecting personal information is for scheduling sessions, communication, and billing purposes.

Confidentiality and Security:

At no time will this information be shared with others.

Disclosure of Personal Information:

Sharing Information:

In the event of an emergency (disclosed or suspected), your information or location may be shared with the appropriate authority to ensure safety. With the appropriate Release of Information shared, some shared information may be discussed with collateral team members.

Legal Obligations:

As a mandated reporter, I may need to disclose information in cases of abuse, neglect, or intentions of harm to self or others.

Client Rights:

Access and Control:

The client can request a summary of case work but will have access to specific session notes.

Duration of Information Retention:

Client information and notes will be destroyed after 5 years of termination or inactive participation.

Communication:

Communication Channels:

Coaching sessions will be held on a private, secure Zoom link that will be shared prior to meetings and will regularly change to limit access. Clients can expect emails and calls to be answered within 24 hours and calls/texts to be responded to in 1-3 hours when participating in the “on-call crisis” packaging.

All parties involved are expected to participate in sessions professionally and respectfully, maintaining civil communication, particularly when addressing a co-parent or caregiver.

Clients will be made aware of updates via addendums sent as PDFs via email.

Governing Law:

This Contract shall be governed by and interpreted in accordance with the laws of the state of Minnesota. Any disputes arising from this Contract shall be subject to the exclusive jurisdiction of the courts in the state of Minnesota.
